Why PCOS Makes Getting Pregnant Harder
The mechanics of conception have three steps: an egg has to mature and release from the ovary (ovulation), sperm has to reach that egg during a narrow fertile window, and the fertilised egg has to implant in the uterus. PCOS interferes almost entirely with the first step.
In a typical cycle, rising oestrogen from a growing follicle triggers a surge of luteinising hormone (LH) that ruptures the follicle and releases the egg. In PCOS, this signalling is disrupted in two ways: baseline LH is often chronically elevated (which desensitises the normal surge mechanism), and elevated androgens plus insulin resistance interfere with the maturation of any single dominant follicle. The result is that many cycles pass without a mature egg being released at all — this is anovulation, and it is the single biggest reason PCOS is associated with infertility.
Everything else about fertility is usually fine. Egg quality in PCOS is generally comparable to non-PCOS women of the same age. Miscarriage rates are modestly higher (largely mediated by insulin resistance and obesity, both of which are treatable). Uterine receptivity is normal in most cases. If a woman with PCOS ovulates, her chance of conceiving in that cycle is roughly the same as any other woman her age — the problem is that she may only ovulate a few times a year instead of twelve.
Ovulation Tracking When Cycles Are Irregular

Basal body temperature (BBT)
Your resting body temperature rises 0.2–0.5°C the day after you ovulate, because the corpus luteum starts producing progesterone. This rise is small and easily missed unless you measure carefully — a dedicated basal thermometer (0.01 degree precision), taken orally the moment you wake up, before getting out of bed, at the same time every morning. Chart the readings and look for a sustained shift upward that lasts more than three days.
BBT confirms ovulation happened but doesn't predict it in time to time sex. It is diagnostic, not tactical — use it to know whether you ovulated this cycle at all, and to identify patterns over 2–3 months.
Cervical mucus observation
As oestrogen rises before ovulation, cervical mucus becomes clearer, stretchier, and more slippery — it should feel like raw egg white. This is the most fertile mucus and indicates ovulation is approaching (usually within 1–3 days). After ovulation, mucus becomes thicker, cloudier, and less abundant.
Cervical mucus observation is more predictive than BBT and doesn't require a thermometer. In PCOS, the pattern can be less clear because oestrogen can fluctuate for extended periods without triggering ovulation — you may see fertile-quality mucus multiple times in a cycle without a corresponding ovulation. Use it as a signal to increase sex frequency, not as a single-day prediction.
Mid-luteal progesterone (blood test)
The gold-standard confirmation of ovulation is a progesterone blood test taken 7 days after suspected ovulation. Progesterone rises sharply after ovulation and stays elevated for about 10 days; a level above roughly 10 ng/mL (30 nmol/L) confirms ovulation happened. For women with PCOS and unpredictable cycles, one strategy is a weekly progesterone test until you see a rise, then confirm the pattern. It is more definitive than any at-home method and worth the small cost and inconvenience if you have been trying without success.
Why OPKs Mislead in PCOS
Ovulation predictor kits detect the LH surge in urine. In a woman with a typical cycle, baseline LH is low all month and rises sharply for 24–48 hours before ovulation — the kit turns positive during this surge, and ovulation follows 24–36 hours later.
In PCOS, this pattern breaks down in two ways. First, baseline LH is often chronically elevated (part of the hormonal signature of the condition), which can produce positive-looking results throughout the cycle even without an actual surge. Second, PCOS commonly produces multiple mid-cycle LH peaks — the body attempts to surge, fails to trigger ovulation, and tries again. Each of these can register as a positive OPK without any corresponding ovulation.
The practical consequence is that OPKs in PCOS produce a lot of false positives, and couples often exhaust themselves timing sex to a positive test that didn't lead to ovulation at all. If you use them, treat a positive as suggestive rather than confirmatory, and always pair with either BBT (to confirm ovulation happened after) or a mid-luteal progesterone test.
Sex Timing Strategy
The standard fertility advice is to have sex during the fertile window — the 5 days before ovulation plus the day of ovulation. This works when you can identify the window. When you can't, it doesn't.

Sperm can survive in the female reproductive tract for up to 5 days in fertile-quality cervical mucus, so this frequency ensures live sperm is present whenever ovulation happens without requiring you to predict the exact day. It also removes the mental load of tracking, testing, and "performance sex" that couples find corrosive over months.
Daily sex is not better and adds emotional strain. Every-other-day is sufficient for fertility purposes. If you know your cycle well and can identify a fertile window (either through cervical mucus becoming fertile-quality, or through medical ovulation induction), then time sex from about 2 days before through the day of ovulation — but for irregular cycles, the every-2-to-3-days pattern beats every timing strategy.
Common Mistakes Couples Make
Patterns worth avoiding, based on what fertility clinics see repeatedly:
- Waiting for a positive OPK before having sex. By the time an OPK turns positive, you have 24–36 hours until ovulation. If you have not had sex in the days before, you are relying on freshly deposited sperm to be in place — workable, but tighter than "sex every 2–3 days for the last week." And if the OPK is a false positive, you have missed the actual window entirely.
- Only having sex during the "fertile window." With irregular cycles, this window is not identifiable in real time. Restricting sex to it means missing many ovulation events.
- Assuming a period means ovulation happened. A period simply means the uterine lining shed. In PCOS, breakthrough bleeding without ovulation is common. Only a temperature shift, cervical mucus pattern, or progesterone rise confirms ovulation.
- Ignoring the male partner. Roughly 40% of infertility cases have a male factor. If you have been trying for 6+ months with no result, a semen analysis is a low-cost early step that rules out or identifies male factors before anyone assumes PCOS is the entire problem.
- Optimising every variable at once. Timing, tracking, supplements, diet changes, exercise regimen, stress management, appointments. Trying to change everything at once is exhausting and burns out couples within 3–6 months. Pick two changes at a time and give them 90 days.
When to See a Specialist
The standard rule is to seek fertility evaluation after 12 months of trying if you are under 35, or 6 months if you are 35 or older. This rule is for the general population. For women with known PCOS, the 2023 international evidence-based guideline recommends earlier evaluation — because months spent trying while not actually ovulating are wasted months, not "still within the normal range."1
Practical timing:
- See your OB/GYN or a reproductive endocrinologist right away if you have no periods at all (amenorrhoea), or fewer than 6 periods per year. There is no reason to wait — ovulation is almost certainly not happening on its own, and time is being lost.
- See a specialist after 6 months of trying if you are under 35 with irregular cycles (more than 35 days apart or unpredictable). Six months is the compromise between "give things a chance to work naturally" and "don't waste a fertile year."
- See a specialist after 3 months of trying if you are 35 or older — egg quantity and quality decline faster after 35, and the diagnostic workup itself takes weeks.
- See a specialist immediately at any age if you have known additional factors: previous fertility treatment that didn't work, endometriosis, uterine surgery, or a partner with known low sperm parameters.

For most of the last few decades, clomiphene citrate (Clomid) was the standard first-line ovulation induction drug for PCOS. That changed with a landmark trial published in the New England Journal of Medicine in 2014, which randomised 750 women with PCOS-related infertility to either clomiphene or letrozole over five treatment cycles. The letrozole group had a live-birth rate of 27.5% versus 19.1% with clomiphene — a substantial and clinically meaningful difference, particularly pronounced in women with BMI above 30.2
Letrozole works differently from clomiphene. Clomiphene blocks oestrogen receptors in the brain, which tricks the pituitary into producing more FSH to stimulate follicle growth — but the receptor-blocking effect can also thin the uterine lining and reduce cervical mucus quality, both of which work against conception. Letrozole is an aromatase inhibitor: it briefly suppresses oestrogen production, which prompts the same FSH increase without the anti-oestrogen side effects on the uterus and cervix.
The other options remain useful in specific situations. Metformin is often added if insulin resistance is significant, or used alone in some cases. Clomiphene is still an alternative if letrozole is unavailable or unsuitable. Gonadotropin injections (higher-intensity ovarian stimulation) are a second-line step if letrozole doesn't work. IVF is generally reserved for cases where ovulation induction has failed or where other factors are present, though it is highly effective for PCOS when needed.
One important note: letrozole is FDA-approved as a breast cancer treatment, not as a fertility drug — it is used off-label for ovulation induction. This is standard practice worldwide and recommended by every major guideline, but it is worth knowing so you are not confused if you read the packaging.
The Role of Weight and Lifestyle
For women with PCOS who are at a healthy weight, lifestyle interventions are supportive rather than transformative — you are already in the range where the metabolic machinery is doing what it should. For women with PCOS and obesity, the picture is different. Modest weight loss is one of the most effective single interventions for restoring ovulation.
The BAMBINI randomised controlled trial, published in Human Reproduction in 2026, followed women with PCOS and obesity through a structured weight-loss programme and tracked resumption of ovulation. Findings:
- Each 1% reduction in body weight was associated with a 5.6% increase in the odds of ovulatory recovery
- Roughly half of anovulatory participants who lost 2–5% of body weight resumed ovulation
- No upper threshold of weight loss was identified — more loss continued to help
